Following is the list of all of the required documents from my personal experiences at Chaeng Wattana. I realize you are applying in Pathum Thani, but there may be some overlap that could be useful:
1 - Form TM.7 Completed and Signed - 2 Pages - Front & Back
2 - One recent 4X6CM Photo Of Yourself – Attach to back side of form TM.7
3 - A letter from your bank certifying your current account balance. Letter to be issued and dated the same date as the day when applying for the extension of stay.
4 – Update passbook at the bank on the same day as applying for the extension of stay and create a photocopy of the updated account balance to give to Immigration. Also, photocopy the front page of passbook showing your name and account number and photocopy any pages in between containing any deposits and withdrawals for the past 12 months.
5 – A certified (signed and stamped) bank statement issued by the bank for the 12 month period starting from the date that the last 1 year extension was applied for until the present date.
6 – Photocopy of the first page of the passport and photocopies of all passport pages containing visas stamps and passport entry and exit stamps pertaining to the current visa.
7 - Copy of the current receipt from immigration for form TM.47 (90 day notification of residence address in Thailand).
8 – Hand drawn map showing directions to your residence.
9 - A single page form that only says "Statement" at the top (provided to you at immigration).
10 - A single page form numbered STM.2 entitled "ACKNOWLEDGEMENT OF TERMS AND CONDITIONS OF PERMIT OF TEMPORARY STAY IN THE KINGDOM OF THAILAND". (provided to you at immigration).
11 - A single form entitled "The Acknowledgement of Penalties for A Visa Overstay" (provided to you at immigration).
12 - Copy of House Registration (if you have one).
13 – Copy of Pink Government ID Card (if you have one).
*All of the above photocopied documents you submit need to also have your hand written signature somewhere on the photocopy (preferably in blue ink).
No need for a copy of a rental agreement (lease)? In some offices they require that.
I'm also interested they now require a drawn map. It's been a few years since I did my extension at CW and they never asked for a map of any kind but in Koh Samui they have for quite a while.

Went today, 27 days before expiry date. No problem. My hotel checked with immigration yesterday who said the rule is it’s ok up to 30 days in advance. But they wouldn’t let me pick my passport up later the same day even though I applied in early morning. Last two years they did. This time she said come back “around 24” which is 8 days from application.

I wasn’t really thinking that, I assume they would do that anyway. But then is there no point in making the online appointment? I’d think the idea is that if there’s a lot of people just showing up they’d give you a queue number ahead of those without an appointment …?

Has anyone recently used the online appointment system? As I recall it wasn't really working last January but maybe it's good now ...?

Just checked my records, I actually went last year January 16 ahead of a February 12 expiry date so that's 27 calendar days (or 28 if they're counting the first and last dates as they sometimes do for other purposes). They accepted it without any issue. Maybe I was lucky.... ?

The vast majority are 30 days in advance.
So if 30 works for you at Samui then all good.
Thanks. Small point, maybe splitting hairs but any idea if they count both the start and end days? Like if your expiry date was the 30th and you went on the 1st we’d usually say that was 29 days ahead but I guess it’s possible they’d say it’s 30? (And I assume we’re talking calendar, not business days here).

Helps if you have eg a flight ticket and "story" of why you need the extension early.
When I used to do it in Bangkok (Chaeng Wattana) some years ago, it seemed you could do it as early as 45 days ahead of expiry. Not sure if that's still the case. I know each office has its own variations. This year in Samui 30 days would work for me but it would cause difficulty if much less than that.

I’m going soon for my annual retirement extension of stay (based on non-OA visa). I‘ll be doing it in Koh Samui. My recollection is you can apply as much as 30 days before expiry. Any recent experience on that? Can I go even a little earlier?

Does the current Samui Form TM.7 (extension application) still have a field for entering your Form TM.6 number? That form is now abolished so on arrival I didn't get a number.

Interesting suggestion. Did you scan them with a proper printer/scanner or use your phone camera?

I once filled in everything from home using downloads, I had to redo it all at the immigration office in their forms. Now I come a day, or a few days, in advance and collect the form.Thanks but as I said, it's not convenient for me to go all the way up there (plus will cost taxis both ways) just to get forms in advance. Was hoping somebody might somehow have them. Or that maybe there's a way to convert the one-sided pdf to two-sided for printing but I don't see how on my computer. If not I guess I'll just head to the office early the day I apply and fill them out manually there.

Does anyone have a pdf or good printable version of the Samui Immigration office's own double-sided application Form TM7 (extension of stay) and TM8 (multiple re-entry)? I know they tend to reject the single-sided version available from the main government website, and I'm staying far from the Samui office so it's inconvenient for me to go a day early just to pick up those forms.

Has anyone recently used a hotel (long stay) as residence for retirement extension in Samui? Wondering if Immigration is accepting a hotel booking as “lease/rental agreement”. (The TM30 and other residence documents are manageable.)

I had visit by two kind policeofficers one morning some years back – they had a long list of foreigners to check, if the lived at their given address; they showed me the list – saves time for police to visit foreigners, when you prove it yourself. It also saves you from a visit by the police.
No matter what we expats think about weird rules at Samui Immigration, we have to follow the rules to get an extension of stay, so much more easy, just to do what they ask for...
If someone has gone to the trouble to fake all the other residence documents (TM30, lease, etc) why couldn’t they just go to their fake residence and take a pic of themselves in front of it? It doesn’t really prove they live there. It’s in fact easier to fake that than the other documents.

The province of Ontario now has a law that limits use of foreign licenses to 90 days from last entry into Canada. If staying longer you need an IDP which makes no sense, I think, but that’s the law.

Thanks. The whole thing seems odd because from what I read, the point of an International Driving Permit is to provide an English translation for non-English language licences. Since Thai licences already have both Thai and English, what is the purpose?

1. Do you have to print the photo or is it OK to have it in your phone and let them take their own pic of it from your phone?
2. Is this document now on Samui's official list? Does anyone have a current copy of that list to share?

It might depends on the offices but for me, in Phuket, as long as you have a valid 5 years Thai DL, there are no other requirements (only regular copies passport, visa pages...and certificate of residence from immigration).
No test, nothing, just provide your original 5 years Thai DL + requested copies and photos (no digital photo on the international DP)
I got few Thai international DP in the last years, being on Non O 1 year multiple entries or the last one in June 2023 on Non O + 1 year extension retirement
Thanks didn't see your comment until now. That's all good except that the International DP is for 1 year only. What a silly limit. So you have to go back every year, and it might expire while you're still travelling abroad. Do you know if you can get the International DP at the Dept.. of Land Transport in Koh Samui?

You should be able to use a guest house TM30 for a retirement visa. Depending on the office they may also want a copy of a rental agreement and the blue book and ID of the owner or manager of the guest house.
