i got an appointment from expatvac for my first covid pfizer vaccine next monday but on registration i couldnt say that i have acute gout high blood pressure and i am paralysed down the left side do you think there will be a problem i am 52 years old or do i have to say on form when i go for vaccine

i live in pattaya and i am disabled and cant walk to much and just wondered who i will have to contact or if possible if someone medical could come to my house to give me covid vaccine ide prefer astrazeneca or pfizer vaccine thanks
