We've just delivered three weeks ago in CM.
Flipping through this thread most info is outdated/non-sense and maybe 5% is ok. I'll be happy to share our extensive research comparing government and private hospitals in CM, the ups/downs we had at both and why we finally decided to deliver @ a private hospital (will also apply to CR) in a call. As I have posted elsewhere, CMPHO are working double duty, their own jobs pre-pandemic as well as spending a full day working on Covid 19 issues. So, take a breath.
Second this - the folks @CMPHO are doing great work. Of course we're all aware that vaccine doesn't grow on trees and with this example it also shows that offering qualified information is still the best way to calm the masses.
It's called crisis managment...

This option mainly caters to German speaking guests, but what do you have to loose speaking to the owner/founder Martin Woodtli. The establishment has received world wide recognition and for the care given (i.e. 3 caretakers per guest), the prices are very moderate. I've had a look there myself and was more than impressed...

From my own circles, I know that the Chinese were already informed by the Consulate General in Chiang Mai that this will take place about 3 weeks ago.
At the same time I called the German Honorary Consulate to be told that they know nothing about foreigners vaccination plans and "it would be better to follow the Thai news".
The German Embassy didn't even bother to pick up the phone for days and when they did, their reply was "to wait calmly while the Thai government will handle things".
Say what you want, but if our embassies were half as active as the Chinese, we would have a much better standing here.
Not only in regards to the vaccination...

Because most countries will not allow entry with a passport validity <6 months, my formula is:
6 months + 3 months (delivery) = 9 months before expiration

I was explained the criteria is either >60 or serious diseases for eligability.
As far as the assessment, for Thais, it seems taht is the reason that they need to disclose their medical history to the government. For foreigners, your guess is as good as mine...

@jackdd They did not mention a differentiation between Thais and foreigners, only that I am currently not eligable for either >60 or underlying problems.
Fair enough for those at higher risk than I...

I suggest you don't wait for them to do anything to help you, because you might die before they lift a finger to assist.

Upon sharing my experience about registering (or better trying to register) for the Thai government vaccination rollout @orang37 kindly suggested to share this in the CM forum, as well:
The first hospital I visited was Mae On Hospital (closest to home), where they were very friendly, but also very clear about the >60 rule and the Thai first clause. The staff there know me and were very apologetic, but also very limited in their "space to move" from policy.
We then contacted Sripat to be referred to Maharaj, as Sripat will not participate the vaccination. At Maharaj, we were told that in this phase that only high risk (>60 etc.) persons are able to register.
That was also confirmed at Nakorn Ping Hospital.
Contacting Bangkok Hospital Chiang Mai (just to cover all bases), we were informed that there is currently neither a plan nor a waiting list for "private" vaccination, parallel to the government's rollout plan.
I must say though, that everybody we have spoken to has been very kind and understanding of the situation and helpful. Our local Mae On Hospital has offered to call us directly as soon as there is a change and we can register.
Meanwhile, this morning, I called the CM German Honorary Consulate only receive my anticipated answer: "We are sorry, but we have no information about the registration of foreigners - but please stay safe"
In that sense, stay safe, stay calm and stay friendly...

Either that or they have been clearly briefed to "politely" let you move on, considering the recent flip-flops of private procurement of vaccines through companies and private hospitals.
But then again, that is all speculation...

@Brierley Yes, that is my understanding: >60 and/or underlying conditions.
I am not sure where to differ between chronic and underlying conditions, as the latter are clearly defined by the policy. Best you call them or go there directly - English speaking operators are available.

Any hospital taken the lead is purely speculative in my opinion, as we don't know from whom and at which level, national policies are translated into provincial/regional tactics.

I can confirm visiting 3 hospitals in Chiang Mai area that currently they have 2 conditions for registration:
1. >60 years of age and/or underlying conditions (diabetes, stroke, etc. )
2. and one district hospital telling me politely in Thai: Thai people first (kon thai goon)
