having  just  spent 8  days  unable  to  walk  due  to  muscle/ligament  damage  in my  back  i  sympathise  with  the  boredom...................never  mind  ive  managed a concerted  effort  to  slag  of  thailand  this  week  and  gain  bonus  points  in  the  process.............laptops  taken a beating  though

My grandmother has Angina heart condition as well.  A few years ago my grandmother wasnt feeling too well and asked if I could take her to the doctor and help translate as she was a imigrant and not too fluent in English.  When we arrived at the medical center the nurse asked us into the doctors room and wanted some background info about her pre existing conditions.  So she asks nan and my nan comes close to her and whispers that she has vag.ina.   I had to look away from the nurse not to burst out laughing.

No offense taken. Not everyone can appreciate the merits of golf.

They do sell cotton buds in supermarkets, you know. Although perhaps you prefer a large deposit of ear wax to mute the conversation of your American GF.

OK. Some serious advice.

 

You only need to be in hospital for a maximum of 3 days after your stent is fitted. Any more and the hospital is milking you. Good to have a young lady to tend to your needs on discharge. No sex for a few weeks but you can do a "Bill Clinton" as soon as you feel up to it. It's a stress reliever, good for you.

 

List of drugs you need to take:

 

1. Clopidogrel. Blood thinner, stops further clots forming. Take for a max of 12 months.

 

2. Aspirin 75mg for life. Same as above.

 

3. Beta blocker. I take Bisoprolol 2.5 mg for life. Regulates heartbeat. Higher doses may be prescribed.

 

4. Ramipril. 10mg for life. Lowers blood pressure. You may be started on a higher dose.

 

5. Omeprazole. I take 20mg in the morning. Was originally prescribed 20mg twice daily. Suppresses the formation of stomach acid. Necessary to counteract the side effects of some of your other drugs.

 

6. A statin. Lowers cholesterol. I was originally prescribed Simvastatin but got leg cramps at night. Need to try different statins to avoid this. I now take Rosuvastatin 5mg.

 

You should also always carry a bottle of Glyceryl spray with you. If you feel your symptoms return (chest pain) spray under your tongue, up to 3 times at 30 second intervals. Call an ambulance first. Must not be used with ED drugs.

 

You should be on all of these drugs now, and only these drugs. Any variation and tell the doctor that you have consulted your personal cardiologist in the UK and this is what he has recommended. Feel free to PM me.

 

The most important advice that I can give you is get yourself on a plane back to the US as soon as you are cleared to fly. Consult a decent cardiologist when you get there. I believe that there are some quite good ones in the US, almost as good as me!

I guess you were relatively young, if I got out of commission for 2 1/2 years I'd resume duty at 69. At this age more than half of us content themselves with fond memories... So, I'll stretch Active Surveillance as far as I can.

I was 55 at the time. Initially, I felt the same as you, but eventually felt some stirrings. Attended a specialist ED clinic where I was taught to use Cavaject. I now get a continuous erection for 4 hours at a time and can service a fleet of maidens at one sitting. Eventually, the Cavaject became less and less necessary. I now only use it 50% of the time.

 

I have a friend who is 81 and still uses Cavaject. Loves the BJ bars in Pattaya.

Thats a standard US regimen IIRC from my friends 3 stenter. I will be asking the Doc on Monday about a Beta Blocker, although the tendency in the USA is NOT to give beta blockers in the absence of an true Infarct.  But, as soon as my insurance claim is finalized, Im Fed Exing my Records to the US and having my folks give them to my Moms cardiologist at Duke University for a consult, even has the CD of the procedure.

 

Interestingly enough, I lucked out with the Docs. The guy I interacted with and who I guess did the initial hands on was Dr. JIranut Cholteesupachai, who can be seen here:

However, the guy that did the stenting was this Doc, who I guess is "the Man" here in Thailand, and is the "Professor" to all of these Docs doing intervention Cardiology. He is partially US educated and US Board Certified, speaks impeccable English, and by looking at his stuff on line, he is like a big time expert. He is the Head of the Cardiac Clinic at King Chulalongkorn Hospital, works at Bumrungrad too.

So I for one feel lucky I got a good one. Hes VP of the Cardiovascular Intervention Committee of Thailand on top of all that. Pretty cool guy too.

 

If you are being a medical tourist to get the ticker fixed, or just feeling the beginning symptoms, I sure as hell would reccommend Dr. Buddhari!

 

Im going to be very curious as to what the US Docs say about the procedure. MY Moms Cardiologist is a Professor at Duke Med, which is of course one of the tops in the US.

 

Anyway, its all moot, they arent going to clear me to fly as I am told at least for 30 days ish, so I will probably go back in March (30 days early), depending on what they say and the dudes in the US say.

 

They did not give me Nitro or Glyceryl. I havent had a stitch of pain since they pulled the tubes out.

 

Are you really a Cardiologist? Want to look at my CD ???? 

????  I cant stand it ????

1. Irbesartin has more side effects than Ramipril and is usually a second choice by cardiologists. However, if you don't suffer the side effects, it does just as good a job.

2.Prasugrel was originally developed in India and has a greater likelihood of causing bleeds than Clopidogrel. Not normally prescribed for our condition in the UK. Having said that, it's just as effective.

3. Baby aspirin = 75mg. 

4. Lipitor is Atorvastatin. One of the best statins....if it doesn't give you leg cramps at night.

5. Omeprazole. I too was on it for several years before my attack. Originally prescribed for a hiatus hernia. Half of Thailand are on it to treat "chilli-itis".

 

Surprised that they didn't prescribe a beta blocker.

 

Very surprised that they didn't give you a Glyceryl spray. In the weeks following my stent being fitted, twice I got chest pains, panicked and used the spray. Was rushed to hospital but both were false alarms. Better safe than sorry.

 

Only joking about being a cardiologist! However, my daughter is a senior nursing sister in A&E (ER). Her husband is an A&E doctor. My daughter was present when I had my second an 3rd attacks. Also had my stent fitted at the hospital that she works at. I regularly email them for medical advice.

 

Also, it's surprising how much you learn when you've had a life threatening condition.

Actually, ARBs such as Irbesartin are now first line in the USA as the older ACEs such as Ramipril are falling out of favour. I couldnt tell you the specifics of why in my case one was chosen over the other.

I thought Prasugrel was Japaneses. Im sure I will end up going with Plavix when I am back to the USA because of the generic factor, I better speak to the Doc about that.

Lipitor is now generic in the US ans as to the Omeprazole, been eating those for years as I have a hiatal hernia.

Me too and Im going to ask about the Beta. However, US doctors are moving away from them

 81 in the US. I have a wad of it with me. The Aspilets (chewable) they gave me are 81s

 

In going over this with you, it is apparent that the Docs in my case a following US protocols, which makes sense since Dr. Buddhari is US trained. The keys here are the ABS instead of the ACEs and the non use of Beta blockers. But I do need to discuss the Plavix vs Prasugel issue. Maybe I can get them to get me at least a 6 month supply here so I dont get raped back in the states.

 

My diagnosis was NSTEMI. Good thing they got it at 90%. And because the rest are clear, if I do my part, I should last another 3 weeks ????

Am curious,  did your insurance company have you get a basic check up when applying for health insurance?  Did you know you had high cholesterol or high blood pressure before this?   or did this hear attack/near heart attack just come on out of the blue after a night of drinking?

No check up and I really dont have super high Chloresterol by US standards. My blood pressure has always been good. 

 

Never been treated for anything other than shingles in the past year.
