They do so many they already know what you need. Its ~ 200 for the letter and 100 to deposit into your a/c. They will photo copy that last page showing your activity. One small tip. With your form for extension already filled in walk passed desk where the crazy folk are collecting forms..inside desk to collect your ticket #, then go to bank. The bank gig might take 15 min. Their location finder shows a branch at Laksi IT Square. Quite a a bit off from government complex and it's a shopping mall micro branch opening at 11 AM.
