Usually these kinds of mishaps are a caused by pushing the accelerator instead of the brake.  Comes back to driver training (or lack of). A good driving instructor installs good driving habits. I was taught to drive an automatic transmission car with my left leg tucked under my right leg so I never had 2 feet near the pedals. People here don't seem to have licenses half the time, let alone driving lessons.
