Isaanbiker
You can have your surname if you both want that. The birth certificate will carry your gf's name and they'll ask her who the father was. If she's telling them that you are the biological father, then your name will be on the birth certificate as well. The baby can't have your last name without a marriage/certificate. NanLaew
The OP should make the effort to get his family name translated into written Thai by a recognized translation service. Leaving it to what a hospital birth registrar thinks it sounds like can cause issues later. Once that written translation is used, make sure that the exact same name as written is used on subsequent forms and documents such as Thai passport applications, ID cards, forms for immigration and the like. Good luck with the new arrival. Just like to add that the OP should check with the hospital where the birth is planned to take place to make 100% sure that his name can be used for the birth certificate. Some smaller, provincial ones may balk at the farang name as Issanbiker has suggested. I recall reading that it's not unknown for the name of the new mum's brother or even her father to be used for a birth certificate when the real father is not available or otherwise missing. The Thai regulations require a name for the birth certificate and in lieu of the real father, next of kin is acceptable... like in Kentucky. Birth certificates are issued by the Amphur office, not the hospital.
