I've been told about this form for my Thai National wife that desperately needs to get back to Thailand (Nong Khai) to take care of very sick parents. Here is the URL link (it's long) that we used. She has already received a reply from the Thailand Embassy. I hope this helps everyone. It is my "understanding" it can be used by anyone trying to get back into Thailand with a work permit, etc. Just explain your situation in detail, as the text block is (I think) the most important aspect of the electronic form. She can fly to LAX easily. WE just got a long email from Washington DC Embassy. She has 3 days to decide on a repatriation (sp?) flight. I've copied the flights from LAX here below. Either JAL, Asiana, or Korean Airlines. 200 Thai Nationals per flight. My wife is thinking about the 28th flight by Asiana. She would rather fly JAL. She needs only 3 things. 1. Ticket on one of these flights below through one of four travel agencies dealing with ONLY these flights for Thai Nationals. 2. A signed letter from the Embassy. Easy to get with the included link. 3. A Fit-to-Fly document. Easy to get here on Maui, Hawaii. Getting the actual Covid-19 test is different. The FTF document can be done my her PCP with HMSA here on Maui.
