My wife had one made to order for export in Phuket Town a few years ago. This particular shop was on Soi Talingchan, just 100 meters or so off the Phuket Road traffic circle. For a number of reasons, I didn't go ahead with it, and didn't even go to see this guy, but not because of concerns about his skills. He made this for my friend: The quality looks quite good, and I expect he could make your design as well. I don't have a name, but he has a shop near the Bangkok Phuket Hospital as shown in the map below. Apparently, he's always working outside it, surrounded by BBQ bits and pieces, so hopefully you'll find him. Good luck! ????
