Antibiotic suggestion please

this question is for my friend. Before some of you starts let me say im happily 9 year married. So its not me. And also its not the point me or not. Thanks, here is the situation:

had sex with a lady (not a hooker) then after a week he noticed that he has a pain and some burning feeling when peeing. Then few days he told me he literally cuming blood. No white color sperm but only blood when he cums. made a quick web search and it turned out that it can be sometimes. and should be ok in 2-3 week time. Called the lady and nicely asked if theres any problem with her.. she told him that she had a bacterial infection few weeks ago but should be ok now. Well obviously he got it. after 2 weeks blood is gone. no more blood. But when he cums there is still paint inside the penis where sperm coming. i dont know the medical name of that part. So at this point he has no blood no liquid coming out from the penis BUT has a pain when cuming and also pain when he touches close t the head of the penis. it hurts where the sperm comes out. that vein part (dont know the medical name) Also the pain and the burning feeling almost gone too. He said he pees more then usual. Like 4-5 times a day now which was 2-3 times a day. i dont think its prostate because he got this after the sex. and it seems its getting better but real slowly.

well obviously he has this bacterial infection. I dont think he can use Zithromax because he hasnt got any yellowish liquid coming out. But i know its one of the best antibiotic for penis.

Can anyone suggest an antibiotic name for him please?

thanks

Pharmacies will recommend 2g Azithromycin, boxes of 1.5g are 160 baht from Fascino. You should also get a Ceftriaxone injection, 250mg should be enough but now 500gm is recommended. 500 baht for 250mg

thanks i will tell him.  Zithromax is the other name of Azithromycin if im not mistaken.

I take Amoxicillin 500 at the first signs of any bacterial infection 40bht (not if you're allergic to penicillin).

If 5 days of that doesn't work, I take Cipro 500 for 5 days 100bht (no exercise as it can cause Achilles tendon tears).

So far that's always worked.

PS. you should never 'wait and see' as your friend has done, as it gives the bacteria time to work it's way deeper into your body.

I strongly recommend your friend get tested by a doctor or clinic. There are many clinics now that do full STD checkups that are not very expensive. Self-treating is not something one should attempt because one can become asymptomatic and still carry the bacteria and give it to others. Get tested.

For something that's not upper respiratory / ENT I would use Dicloxacillin or Augmentin which is Amoxicillin mixed with something known as 'clavulanic acid' (makes it a lot more effective, think of the clavulanate as a catalyst) for things like skin infection, UTI, etc.

Just a suggestion.

Even the doctors here often don't bother recommending getting tested. They tend to treat first by trial and error

Shouldn't the doctor decide? I just went through this thread and a bunch of recos for drugs but no one has a name for what it is.

The poor chap has a bunch of symptoms. He should see a doc and get diagnosed to begin with to my mind. His penis is not something for a man to play around with. Well, you get what I mean.

If I ever get in the same situation again I will DEMAND that a culture be done on the sample.

Totally makes sense. And only a doctor can order a culture. And then it's what a 24hr. turnaround to start the correct treatment. So the OP or his friend or whoever should head off to a urologist instead of experimenting on his cock.

See a doctor.

From the 2015 Sexually Transmitted Disease (STD) guidelines, the CDC recommends treatment for a gonorrhea-chlamydia coinfection with azithromycin (Zithromax) 1 gram given orally in a single dose, plus ceftriaxone (Rocephin) 250 mg given intramuscularly as first-line therapy.Mar 31, 2564 BE

A clinic or hospital and give the injection.  Pop the Zithro and get the shot usually all gone in a few days.

Doxycycline also can work well on a 10 day treatment.

You're quite right and I find that bloody ridiculous.

I had some sort of UTI and went to the specialist at the big hospital here and he started me on Amoxicillin, which didn't do the trick and two weeks later I was back to see him, so he gave me Augmentin, which also didn't do the trick, so he then put me on ciprofloxacin, and by the end of the second week not only was I feeling very sick, I had tendon problems, so I went back to see him and as I had done many times in the past, demanded he take a sample and culture it.

Because I questioned his "expertise" he ordered me out of his office saying that he could do nothing for me, and this when I was quite sick and feeling not well at all.

So I saw the second urologist at the same hospital and begged him to do a culture, which he did anyway, and it turned out to be a bacteria which needed to be treated with an expensive antibiotic, delivered intravenously every day for two weeks, which did the trick.

If I ever get in the same situation again I will DEMAND that a culture be done on the sample.

remember the name of the antibiotic?

Yes it was a Carbepenem which can only be given intravenously.

And for the record this has happened to me twice – – and the second time was when I was administered all sorts of antibiotics for a problem with my prostate, so a cystoscopy was carried out and I believe I got an infection from that, because when the culture was finally identified, it was E. coli ESBL (extended spectrum beta-lactamase) which is either hospital acquired, or as a result of too many of the wrong antibiotics.

Yes I have become a bit of an expert on this, and I think the situation here as regards urologists is absolute cr@p.

Amoxicillin is a very poor choice against STDs. The only thing it is really effective against is Syphilis and that is an incredibility rare disease (even then Doxycycline should be used in cases where Penicillin injections can't)

Doxycycline or Zithromaz would be more effective against the common STDs.

Anyway, self diagnosis/treatment is dumb.
