Lots of people accidently lose this card , even though its stapled in to your passport. Just go to the Immigration office at Soi Suan Plu (South Sathorn Road in Bangkok) with the passport (assuming all stamps in the Passport are legit!!!!) and you will get a new TM card based on the last computer entry they have.
