I met a guy a few days ago and this topic came up.

He told me his PSA is 220

I asked him 2.20?

He said no 220!!

He is going in next week for a biopsy...

Never heard of it this high.

It is possible. In fact over 200 even. My good friend had a PSA of 62 I believe. The doctor told him it was actually much higher but was masked by him taking Finasteride. After biopsy, one-half of his prostate was positive for PCa.

Yes, I've know guys with PSAs in the triple digits and sadly they all had PCa, often it had already spread.  It's not a good way to go.  The cancer often spreads into the bones of the pelvis and back, making the man bedridden and suffering from extreme pain.  Personally, I don't think it's something that should be ignored, but "watchful waiting", i.e. periodic testing to see if the PSA number is rising rapidly is a good way to go for someone in their 70s and beyond.

Being a prostate cancer survivor, I absolutely feel that one has to do way more to boost the immune system , and detox the body ,apart from watchful waiting!

Essential to get vitamin D levels checked, then up your levels with vit D and K2.
A lot of good info on U tube etc.
Diet is critical, cut sugar etc.

Google HCG test, helps of good info, people need to be pro active in strengthening the body!
This is not going away by itself!

The side effects of the surgery are horrible though.

For sure if I hadn't had the surgery I'd be dead now, but I've said it before, and sometimes living is worse than death.

If you don't mind me asking, what were the side effects TBL?

Just a followup on a guy I met who had a PSA over 200.

He had surgery this week Prostate removed and chopped his nut´s off.

Does not sound like much fun.

He seems to have a great attitude about it all, probably much better then I would.

Geez, doesn't sound good although if he is a positive sort of fellow then he can now join his local choir group as a falsetto!

Sorry , but what is TBL?

For info , brother in law, PSA 220, biopsy Gleason 8.
Stage 4 PCA.
Put on hormone treatment.

Major diet changes.

Had course of chemo.

Now after multiple bone scans.
No trace of cancer and PSA 2.4.

There is always hope.

Usually impotence.
To some people it isn't a side effect that would matter.

Guys I feel impotence is a factor to be considered ,as is incontinence ,that is often a side effect of surgery!

My PSA 12.5 Gleeson 8 T1 tumor .

I elected to have Cyberknife at Beacon Hospital KL.
Google is your friend here!

Now 7 years later, PSA 0.35 nil impotence!

Sex is one of the big reasons I am here.

In my mid 60ś I boom just about every day.

Not bragging I know others same as me.

Sex with Thai girls certainly is nice I have been all over Asia and still rate them the best overall.

Filipinos ain´t bad either.

If I became impotent or had the radical surgery not sure what I would do.

Would be depressed for sure.

Maybe hook up with an older lady here don´t know.

Or head back home without my nuts.

Either way I guess now I try to take advantage of my ability to have lots of sex.

Never seems to get boring but having various girls certainly helps.

Guys the secret is, get PSA/digital checks at least every 12 months.
Now what is important, is not so much the level of PSA, ie 0 to say 5, but what is critical , is to look for a spike, sudden rise.
Ideally then have an MRI guided biopsy, if deemed necessary.
Now once a tumor, and gleeson score of the tumor is confirmed.
At this time , early intervention with Cyberknife radiotherapy ,can work wonders, and totally preserve bodily functions.

Once the tumor is outside the prostate capsule, then a different ball game!
You do not want to go there!

Yes I pushed my luck by trying every alternative fix available.
To no avail.
This then ,to eradicate a growing aggressive tumor ,took a lot of radiation.

Hence the body was 100% functioning, but 4 years later, the radiation spill caused a stricture in the urethra, needing a superprubic bag fit.

Not a big deal, but catch things early and avoid this.
It will not go away.

Scary stuff indeed!

Damn !  Good  info and advice  but   next week I am off to check out some  issues. Not sure  if your post  encourages  or  depresses me . lol

PSA  peak at  6.3 and  follow  up  down a lil  bit  to  5.8. Digit test  indicated  no  nodules, just  good ole  standard  BHP.

But  my aching  back and  bits says  go  again !

Being on active surveillance myself, my diagnostic prostate radiologist told me that regular ejaculations have a positive effect with regard to deleting PCa and this has been seen in large clinical studies. He recommended at least three a week, as well as drinking lots of fluids to, as he said "keep the pipes clean". The theory is that regular ejaculations flush out toxins and improve blood flow in the prostate.

If you had a DRE, ask your urologist for an estimate of your prostate volume and then look at PSA density. Prostate cells make PSA so a larger prostate produces more PSA, and you did say you were diagnosed with BPH. If you are still concerned, you might want to consider multi parametric 3T MRI, although finding a good one with a prostate specialist radiologist in Thailand is not easy.

Incontinence and impotence are almost always a result, although for most men both improve with time.

Since your prostate will have been removed, once you have gotten past the impotence, you will have dry orgasms.

I experience them now taking Finistride and Urief but I have not had my prostate removed.

I did not like them so came off the medications as my PSA was just 0.5 last time it was checked 7 months ago. I am peeing more often but I suppose that is to be expected if I am off the meds?

My own experience (61yrs old, somewhat sedentary lifestyle):  Had some issues with peeing more than 1x at night.  Went in for PSA 1.5 months ago- 13.6 was put on antibiotics (Norfloxin 400mg) 2x/day for a month. 1st week down to 8.6, second week down to 7.2, 3rd week down to 6.5, 4th week end down to 4.4 with a free PSA to PSA ratio of .254.  So looks like prostatitis.  Even the 4.4  might be a bit higher than what can be as I do drink a couple of Lg Changs a day, drink coffee, and am on a combo of high blood pressure medicine where one of the components does kick up the prostate a bit for some (only side affect).  Easy and cheap to go straight to the lab here in CM (BRIA) at about 600 baht a PSA test. The HB combo is quite remarkable.  Was sitting at 161/91, started the combo and 5 days later was at 124/80 and have been around there ever since.  The combo is not on the market yet as a single pill but I can get the double does pills w/out prescription here cheap and cut'm.  Here is a link to the latest findings-  https://www.acc.org/about-acc/press-releases/2018/03/10/10/51/mon-8am-low-dose-triple-pill-lowers-blood-pressure-more-than-usual-care

Finasteride is prescribed for BPH and works by reducing the levels of an androgen, which is a sex hormone. This causes reduction of prostate volume (and reduction of PSA) with the effect of alleviating some of the symptoms of BPH, such as frequent urination. When you discontinue Finasteride, the prostate begins to increase in size and symptoms can begin recurring. From what I have read, Finasteride has the effect of halving your nominal PSA, so it can mask an otherwise high PSA.

One thing to consider is that there is evidence now that Finasteride can increase the risk of certain types of high-grade PCa. Other studies show it can decrease the overall risk of developing PCa at all. Like so many issues surrounding PCa it's sort of damned if you do and damned if you don't.

I'm a little confused. Do I understand that you took 4? 5? PSA tests in 5 weeks? I've never heard of that. Did a doctor in Thailand recommend this? That is most unusual.

I took Levofloxacin 500 mg./day for 30 days in October and November and will be taking my PSA again next week to see how I am doing. I believe I was walking around with prostatitis for 4 years, but it was apparently missed by all of the doctors.

I went into fairly extensive detail in post #66, so I'll try not to duplicate. But, I can only say that radiation vice surgery was the best decision I ever made. I had 119 radioactive seeds (brachytherapy), which apparently kept the prostate as the sole target, as I've had no evidence of radiation damage outside the prostate. Cyberknife wasn't yet available for me, but I think, because of it's superior targeting, that would be my decision today. But, in any event, radiation vs surgery turned out to be a no brainer (as I only could have done worse with surgery, since no evidence of cancer, and the latest PSA, 6 mos ago, .0004).

And, yes a few drips, but no pad yet needed (procedure was 13 years ago). And definitely no impotence, as I long as I take half a 100mg Viagra (and lasting power! Where were these puppies when I was a teenager!)

Anyway, whatever the procedure is you choose, just make sure the guy with his hand on the controls is not a rookie. OJT on your prostate could ruin your whole day -- and life.

According to Wikipedia, Viagra was approved for medical use in the U.S. and Europe in 1998.  I left corporate American in the mid-1990s when Hubby and I started our own small business and I remember thinking how fortunate it was that I no longer had to sit around business team meetings where I was the only woman in the room and had to endure listening to some of the juvenile banter.  I expect that after 1998 Viagra became a favorite topic for some of the locker-room humor.

JimGant -- I suspect that your teenage days may have been long before 1998!

Without doubt, catching PC in its infancy, then electing radiation treatment, will for sure give you the best chance of preserving potency!

Purely because of where the prostate is located, it is impossible to avoid nerve damage, when opting for surgical removal.
Even using the Da Vinci robotic procedure.

Brachytherapy can be an option if the cancer is low grade.

If high grade, HDR Brachytherapy is another option.
Google is your friend here!

Cyberknife is by far the least invasive, with less side effects ,obviously If cancer is caught early.

Nancy, sorry you had to leave corporate 'old boy' America before you could sit-in on Viagra jokes. But, thank you for participating with your insight on erectile dysfunction and longevity on this PSA thread.

Yeah, maybe that's why I can do OK with ThaiVisa, after years of sitting around meetings where "the guys" could get off on juvenile digressions or play games like seeing who could best hit the trash can with crumpled paper.

I was never the object of any inappropriate comments or advances, probably because I was up high enough in the food chain and married, but I knew if I objected to the content I would have been told that I lacked a sense of humor and that "boys will be boys".  At some point thought, I just wanted to say "can't we just get back to solving the problem of how to get back on schedule with the warehouse expansion project" or whatever the subject of the meeting was.
